A simple online clients register

This should be a web based application that shall be used to collect clients info and their vehicles info. clients info should include, name, residential address, mobile number, passport image, signature image, date of birth, state of origin, LGA, Nationality, blood group, Next of Kins details.
The app should have two options for clients vehicle, commercial vehicle or Motorcycle.
info to be collected on vehicle should be Reg number, make and type, name of owner, engine number and chassis number.
After this information is collected, the next is to collect the clients PIN if he has, if not one is given to him and attached to his profile.
This is an annual duty hence, any with PIN means he came for renewal and the print out shall bear renewal. Those without PIN shall get one from the system and the print out shall be New registration.
