A PHP send-email script that works with my form/checklist

I have created a form/checklist for my project using HTML that is styled using JQuery.
90% of this form is checkboxes, the rest being a "title" and "send to email".
I require some one who is good with PHP to be able to create a script that:
1. Sends the information from the form to a user in-putted email. This includes the title of the form, and the additional user added "my items".
2. For the HTML to be sorted out in order to function correctly with the PHP if it an issue with the form.

I can no longer devote any time trying to get this form to work and I am confident that someone who understands the language will have no problem with this.

I will send you the HTML page with everything on it, perhaps I have written the form wrong. I can also send you my attempt at the PHP, although that may be more of a hindrance.

The page is currently optimized for a smaller mobile screen so when designing, I always have a web browser the size of a mobile phone.
