4 step php form with mysql and upload a video field

I need a 4 step form. 1st step: Licence agreement and stuff, checkbox and go to 2nd step: Name, adress, mail, etc... submit go to 3rd step: upload a video (to a folder at the host service) max size 100mb, any format go to 4th step: like and share on facebook. The form must to send to the client a report like "your video is with us" and send to admin all the form data including the link for the uploaded video file.

Dont have any idea how much it costs. Im open to negotiation. Thanks
