16" x 10" bench memorial plaque

I am ordering a 16" x 10" bench memorial plaque for my daughter. Here are the details and I have the images to be used.



background image (bk.jpg) from border to border and photo image (black and heiihte.jpg) situated within the butterflies of other background image (black and whit.jpg)

Below is the text which I hope to use and the placement of the image within the text.

Would you be able to do up the whole thing for me so I can just send it as a jpg to be engraved? If so how much would it cost?
