Revamping a word-document

Hi,

I have a word document which needs to be revamped/reformated.

After OCR conversion and importing as well as exporting it in a translation tool the format was ruined.

Please:
A) Check the document and have a closer look
B) Quote a fixed price and a reliable turn-around time.

I need the document as soon as possible.

I have attached the original .pdf file to show how the design was before the catastrophe.
The word document doesn't have to look like the original .pdf but should be in a readable format / design
