Spreadsheet Creation for Multiple Users

We are looking for someone to create a facility for various people to be able to submit data through an online form which all goes into a central spreadsheet.

We are a radio station with presenters in various locations. After each show we have to collect data about the songs played for performing rights purposes (Artist Name, Song Name, Album, Date Played). Right now our DJ's send the data manually and we copy and paste into a master spreadsheet.

We would like a way for DJ's to be able to fill in a simple form (behind a password) and when they click send it populates a central spreadsheet. We also need to be able to manually add in (import) data to the spreadsheet from our auto DJ system which plays overnight music and outputs to excel or csv file.
