Referral postcard design and copy

I am looking for someone to design a referral scheme postcard which can be included in our fruit boxes that get delivered to corporates.

The design must be as catch as possible and we will also be looking for advice with regards to the type of copy we should be using. We will of course supply some ideas for the copy and details of what we would like to see included.
