Experience Level: Intermediate
We require a skilled web developer for start to finish development of an online survey product to be developed in PHP and mySQL.

The survey cms system would be very similar to other well known hosted survey products available on the internet.

The survey system would consist of a number of modules.
Below is the basic outline. A detailed spec will be sent to the right candidates.

DESIGN MODULE:
Administrators must be able to design the questionnaire and page layouts online.
Ability to setup each page of the questionnaire, enter the questions and select various types of inputs depending on the type of question such as:
Ratings scales
Multiple choice - single and multiple answers
Matrix of drop down menus
Single and multiple text fields.
Comment forms
Numerical text boxes
Date and time
Images
Demographic fields.

Input fields would consist of radio, check box, drop down lists, textboxes etc and various combination of these.
Validation must be applied based on question/input type.

COLLECTOR MODULE:
This is where you manage (collect) respondents who are eligible to complete the survey
Ability to add, edit, delete a collector instance for a survey. Ability to have multiple collector instances per survey.
Ability to add edit and delete respondents to a collector instance.
Send out email invites to respondents
Ability to view number of responses
Would require an email management system to be bolted on.

ANALYSIS MODULE:
Ability to view a summary of the total responses and weighted scores
Drill down to view individual responses
Filter responses
Download responses to CSV

ADMIN MODULE:
Administrator privileges.
The ability for administrators to manage general settings for their survey (set cut off date, max number of responses etc)

Developers based in UK or CH only.
Remote work is acceptable but would require ad hoc face to face meetings.
London based developers preferred but not essential.
