I require an updated "academic" monthly calendar based on an old template

The template I am attaching is what I would like each page to look like and ideally could have accurate calendars (but based on this template - minus the hand written text, I simply scanned a couple pages from last year's calendar and the hand writing is simply what I require the girls to fill out on a regular basis), I'd like to have the calendar beginning in September 2016 through March 2018 ( or beyond). I used to use Microsoft Office's Calengdar Wizard however unfortunately its no longer as easy. Ideally if I could add months inperpetuity as needed but incorporating the formatting/layout provided/text boxes provided that would be fantastic... even more fantastic though would be having you add your creative/artistic touch to this continual calandar (that would be fabulous) - the only limitation is that these monthly academic "calendars" will most likeley be printed in b/w on a laser printer. Let me know if this is something you'd be interested, when you think you could have it done by and what you deem fair for the work required here.
UPDATE: I have received a few proposals which are quite on the high end for what the job actually entails, simply making a calendar (preferably one that has the ability to extend into the future and matching the dates to the correct days of the week. Then simply adding two boxes for comments as per last year'a calendar. I just quickly logged into office 365 and used excel to build the first month in no time at all - I'd prefer to outsource this task but at the price quotes I've received it's far to expensive for our team and I'd just as soon do it myself at these prices.

Matthew, how about a calendar in Excel, showing one month at a time (if you so wish), either pre-populated until 2020 or so, or with the possibility to extend its date-span as necessary, at minimum cost to you?
