COMPLETION OF MEDICAL DATABASE • project to design Microsoft Access forms / web-based forms that link to a Microsoft Access database for analysis.

AIM OF PROJECT:

To research the outcome of treatment with dental implants. To complete the creation of a database :
- input and allow the analysis of data collected on a paper data collection form
- allow reports to be produced to see easily the type patients treated, with which treatments and the outcome of their treatment
- to produce a web-based data collection form allowing data to be inputed by multiple clinicians at different stages through treatment
- this may lead to a marketable database that could be sold to other centres and that is something that I am happy to factor in to compensate not being able to pay the full commercial rate.

I’M STUCK - My hours of creating Access fields, lists etc. has come stuck at the last hurdle the creation of linked forms that work to allow easy data input, despite having self taught myself as much as possible. I have many other projects that need an experienced programmer’s input, so if you prove yourself on this project there will be many more projects that I hope to use this one to get funding for. Unfortunately this project has a limited budget as I am self funding it. Although I am a dentist I am a postgraduate so don’t get to excited I have very limited funds, but am passionate about completing this project as it will help lead to bigger things.

I am sure that you will appreciate that I shall not be providing any patient related information, the centre that it is to be used at and the names of any clinicians involved.

ALREADY COMPLETED:

 Comprehensive paper based data collection form with all fields and option lists

 Tables with all fields and lists of options in linked tables on the paper based data collection. I have created with wizard simple
 Now out of date - Database forms made up of tabs for each section of the data collection form, which I have not updated all of the fields on as I think it may be quicker for an experienced

PLAN FOR PROJECT • I would suggest that the project is undertaken in three parts:

Stage I

 Linked forms - Creation of forms for each section of the paper data collection form, which are linked by common information e.g. hospital number.
 Compatibility - Needs to be compatible with Access 2002 for analysis.
 Forms graphics - Minimum would be layout of paper based data collection form (although should be more compact as options can be drop down lists)
 Predictive fields - Some fields data to be calculated/ determined using previous entries e.g. Implant site order in table always remains same order in review tables; Aesthetic table gives total; PES & WES tables give total at bottom table

I would propose at this stage to test the database to check all forms working correctly.

Stage II

 Controls/ Search/ Reports form - Add form which allows patients records to searched, clinician treating case, creates reports for type of patient, type treatment and outcome of treatment.
 Security • password protect front end (forms) to me
 Improve graphics • insert background

Stage III
 Make database so Web-based data collection form and report writing.
 Advise on where host database form and data files
 Advise if possible to link patient details can be linked to hospital database
