Converting a PDF Form to Online HTML Form and saving the validated data to database

A business school needs a few hard copy forms to be converted to their equivalent HMTL versions, the forms need to be validated and the related script should be functional at cross browser/version level. The validated data will need to be saved into a database using PHP script. The HTML form should be integrable into word press. The structure for database should be clear enough for other developer who will take on the job to be able to write further PHP scripts to use the data saved into the database later on.
