URGENT: Secure .Net/Sql Login Page

Experience Level: Intermediate
This is a very simple job which I need doing within the next 48hours. I have a simple template (html) ( see attached picture ) which needs to be coded to login to a SQL database securely. The first box will be 4 characters ( eg ship, boat etc ). The next box will be 6 digits ( eg 123456 ) and the final box will be up to 7 characters ( eg admin, manager )
Each of the boxes need client side validation. When the user clicks login the three boxes need to be checked against a sql table. In this table will be entries such as :-
ID KEY1 KEY2 KEY3 BOAT ROLE
1 boat 123456 jones HMS FRED sailor
2 boat 234567 timms HMS TOM captain
3 dingy 987654 smith HMS LITTLE sailor
If the login is successful then the user will be directed to a default.aspx page.
Each .net page must also have some code which checks to see if someone has logged in correctly otherwise it will redirect back to the login file. I dont mind whether sessions, sql tables etc are used for this as long as it is a proven technique and is free from sql injection attacks etc..
Please no silly bids this should be really easy for someone with .Net, C# and SQL knowledge and probably something that could be done very quickly as all the technology and techniques are already there.
I dont mind you creating a simple login.aspx page to code and some sql code for me to test on my own server..
