There is a questionnaire with 100 questions, which belong to 10 subgroups.
The questioned are answered on a scale, every step of the scale has a specific value (1,2,3,4,5). Some questions are reverse (one clickes what means 4 for the other questions, but here it means 2).
10 sets of 10 questions (which are distributed seemingly random) belong to one value (summ of all questions scores, 10 to 50).
There are also 5 higher categories where you sum up 5 times 20 of the questions and get a result of 20 to 100.
for those in total 15 summed up scores there is a distribution that tells us how the value translates into a percentile value. (Eg. Set one has percentile 1 for values 10-14, percentile 2 for 14-15, and so on).
The visitor of the website fills in the questionnaire and leaves contact details.
We either need the 15 percentile values. In the end the visitor gets his values visualized in a radar-grafic (Illustrator files available) and finds text based in the results. He should only get the results after someone of our team
Manually approves.
We could imagine 2 processes:
1) as a excel document sent to an email and we manually create the Grafik and creat the pdf in word with the serial function or, ideally,
2) we have a custom pdf or Website created for each respondent where they find their data in our CI
We are open to discuss your specific ideas of realization.
And we are in a hurry, we want the questionnaire up ASAP, not longer than 2 weeks after you get our go.
