Joomla VirtueMart Customised Add To Cart Process

I am developing a online optitions using Joomla 1.5 and the latest version of VirtueMart. The client needs to collect prescription details for each item (of a certain product type) when it is added to the cart.
I would need an additional page when the Add to Cart button is pressed to collect these details and store them in a new database table. Each product could feasibly have a different prescription so the prescription needs to refer to the individual product not the order. The prescription details need to be displayed on the cart page and the customer should be able to click from the cart to edit the prescription for that item. The prescription fields also need to be made available for invoices etc.
The form for providing the prescriptions had already been developed in Javascript for a previous website and this script is attached. The process can be seen in action on http://www.easyspecs.co.uk when you add a pair of glasses to the cart. You will see the "My prescription doesn't look like this" button this allows the user to enter their prescription in one of two methods. There is a simple script which will need to be run in the backend to convert the details to a standard format before storing in the database.
The website development has only just started but this checkout process needs to be in place within one week. Ideally by 23/04/2010.
If the successful bidder delivers on this then there is likely to be much more to follow.
