JQuery Form Builder

We require a jQuery form builder to allow users to drag and drop fields onto a visual form with the intention of generating the form as HTML. The visual form builder must demonstrate the height of usability, utilising the very latest jQuery techniques. The action of building a form must first and foremost allow users to very easily add, edit and remove HTML items on a form. All items can be added via either a click or a drag and drop onto the form. The form builder will be integrated into our ASP.net CMS.

All candidates must be experts with jQuery and have past work examples to proof their expertise.
