General information for the website: We sell business plans and pitch decks
Kind of development: Customization of existing website
Description of every page/module: Our website has a client management system. On this management system, we have a questionnaire that we require our clients to fill out before we initiate the project. Mainly, we have a few goals with this project:
1) Optimize design to be more compatible with design of backend
2) Adapt questionnaire to suit customers from different countries
3) Make form restrictions more compatible
4) Improve functionality.

Description of requirements/features: Design:
Currently, the design of the questionnaire does not match well with that of the actual client management system. I would like for the two to look more cohesive together.
1. Change questionnaire fonts to match the smaller font used on the backend.
2. Minimize spacing between text and textboxes and between questions to make look more professional.
3. Minimize space by bringing some lines together… For example, first and last name can be on one line and the address/city/state/zip code line can be on two lines instead of four.
4. Paginate pages so that they are shorter….. Each section should be on a new page… “Business Contact”, “Business Structure” and “Business Description should be three separate pages.
Country Selection:
Although we do plans for several companies, the Questionnaire really only suits those from the US. I would like instead to focus on three areas – “United States, Canada and United Kingdom”. Some changes will need to be made to the questionnaire itself to suit.
1) Limit “Select Country” to three options, United States, Canada and United Kingdom
2) State/Province should be a dropdown list. It should show different states/provinces options depending on what option the user chose in “Select Country”.. For example, if “United States” is selected, the user will be able to select from a list of US states.
3) Telephone numbers aren’t always in the same US format, so if someone chooses “United Kingdom”, then the phone number box should change to accommodate the UK phone number format.
4) Add Currency markers before each Price, Cost and Amount Value… For example, on page 3 “Financials – Capital Expenses – Startup Expenses”, the Amount section should have a $ or £ before the input box. This currency marker will need to change based upon what currency the user chose on the first page.
Restrictions:
Currently, most of the boxes have some type of restriction… like must be text or numbers, etc… however some of these need to fixed.
1) Outside of the “Business Description”, users should be able to leave boxes blank. If they don’t know an answer, I don’t want it to prevent them from moving along.
2) Zip code should include numbers, and special characters such as “/” and “-“, as well as spaces.
3) Telephone numbers aren’t always in the same US format, so if someone chooses “United Kingdom”, then the phone number box should change to accommodate the UK phone number format.
4) Owners Names and Advisors names should be allowed numbers, spaces and special characters
5) Whenever a restriction blocks a user from advancing, it should alert them as to why.
Functionality:
Lastly, I would like the way the questionnaire “Submits” and “Posts” to be changed. Right now, when it is submitted, it submits exactly as it is shown; still showing the whole form, just not able to edit from the staff/admin side. I want it to post without the form, so when it posts back to staff, it just shows the question, and an answer WITHOUT the textbox behind it.
