Due to the huge response from bidders, please see revised Job description which contains additional details:
1. Build a bespoke salary survey website with PhP and MySQL.
2. Import existing data from old survey site (7000 records x 22 fields)
3. Get the site up and running on the www.Hostway.co.uk VPS.
This is to be based on an existing Salary survey that already belongs to me, but that is embedded in another website.
I have already looked at simply copying the existing web pages to a new website, but it relies on scripts that are in use in other parts of the old site and it is too complicated/messy to edit the existing scripts to work on a new site (At least it is for my current in house web developer!)
To get an idea of what I am looking for:

The scripts for these pages are available if that is any help.
The existing database which has about 7000 entries needs to be imported to the new site.
The new site will consists of a handful of pages:
1. Data entry page- public page for punters to enter their salary details: http://clients.oc99.com/ratesurvey.php
Column of spots for advertising on this page AKA google ads
2. About us page – text about us blah blah we are great etc
Data input on this page for paying clients to register and have full time access to the survey. Name, co name, address, contact details etc.
3. Survey results page http://clients.oc99.com/surveyresults.php (password or cookie required for access)
4. The survey results page needs to have a ‘download’ function that will generate a pdf from the database that can then be downloaded by the user.
5. Admin area page –(password)
allows adding/editing/deleting of entries
Change currencies
Add new job titles
Allow Download of data to Excel or CSV
Admin advertisers on home page- hit counter etc.
6. Admin area- Add clients
7. Error checking script required to stop stupid numbers/garbage being input.
8. Principle is that if a person provides their salary details, they can get a free copy of the salary survey results.
To be built ASAP expected build time- 10 working days maximum.
