Need HTML/ CSS coding for website holding page, design already complete

General information for the website: Holding page only
Kind of development: Customization of existing website
Num. of web pages/modules: 1
Description of every page/module: Relatively basic holding page required. Consists of: Logo, Heading, two different sized bits of text in OTF font, background large text, contact form.
PDF of design attached, all images will be provided as PNGs or other type if preferred. Font files also provided.
Description of requirements/features: - The page has been designed for four width breakpoints - 1025px and above (desktops), 1024px, 1023-751px (tablets and small screens) and 750px and under (all mobiles and small tablets).
- Design PDF includes sizes between these main three - this shows how the site is to respond at midpoints.
- Website will respond to browser window or screen size.

Full setup document is attached.

I just need the .html file with details of folders referenced etc. I can upload via FTP.
