I need a profesisional gamified web based questionnaire design

I am conducting a survey and require the use of gamification in a conventionally designed questionnaire, which aims to appeal and get the involvement of teenagers, who would form part of the participants in this survey.

The questionnaire has already been designed on paper but most importantly, it requires a skilled professional to convert it to a gamified web-based one, thereby applying game elements to different sections of the questionnaire.

I am also particularly interested in a multi device web based questionnaire, to allow for more inclusivity for respondents who may be using different devices to respond to the questionnaire
