Fathers name:
Phone number:
Address:
Email address:
Photograph:
Check box at the end of the document that will say “I agree to abide with the rules and regulations of 5 A Side Futta tournament”
Payment integration of Paytm to be made here.
Once submitted and cleared with the payment, I should get a email in pdf format about the team.
If they don’t use paytm, they should submit cash, once I receive the cash I will sent the confirmation mail manually.

Match fixtures and results
Will need a backend to upload all the relevant details. Following details will be entered:
Date of match
Team 1 vs Team 2
Winner team
Score of the team
Players name who scored the goal with an option to give his photo (the photo could be taken automatically from the registration page)

Gallery:
Will need a backend to upload/delete organize the photographs.
