We are looking for someone to build a bespoke iOS app which has two types of form- one of which is a 10 question survey, the other which is a data capture form with 6 fields.
The survey form has 9 multiple choice questions and 1 free text question. The survey also has a disclaimer with a check box which needs to be ticked prior to the questions being displayed.
The data capture/sign up form has a legal statement with a check box which needs to be ticked before continuing and 6 text fields.
As per attached wireframes and user flow, after the survey or data capture form, the user is displayed a 'thank you' page which prompts them to then complete the other form. Display an alternative 'thank you' form if both forms have been completed.
The app should automatically go back to the home screen if there is no interaction for a given period of time. N seconds during the survey and potentially a shorter duration for the thank you screens.
We require the data from these surveys to be stored separately so we will have a CSV for surveys and a CSV for sign ups. We also need to have an admin interface that allows the user to export these files easily. Our current suggestion is a hidden touch spot that allows the admin to click and then be prompted for a username/password which would take them to the export screen as illustrated within the wireframes.
The data should be stored OFFLINE within the app.
The "admin" area is part of the app and is simply 2 buttons to email the CSV file to the user as per the wireframe.
The ability to export the data in CSV is a must for this. The idea is that when clicking on one of the 2 export buttons it will allow the user to email this file to themselves. Whether this open a compose email window or it simply sends to a specified email does not matter. Please make suggestions for this export process.
