Need a page on website updated for an upcoming event

General information for the website: update summit registration page

Description of requirements/features: We have a summit coming up that needs to be advertised on www.hepatitisfoundation.org.
This is the exact page that needs to be changed: http://hepatitisfoundation.org/NEWS/Summits.html The information in the light blue column on the right side of the computer screen is what needs to be updated. I have included an attachment showing this in more detail. I will provide the event details that need to be applied to the Register Now button in a pdf.
Also, within the pdf, there are 2 Click Here buttons that have coding that will need to be changed to accomodate the Paypal payment code. Need this done ASAP!
