Kind of development: Customization of existing website

Description of every page/module: Only the form/plugin for form needed.
Description of requirements/features: I am looking for help in creating a form on a Wordpress site that produces a few important functions.
Here is what the form needs:
• A calculation sum of particular options in a drop down field and select buttons.
• Multi-part.
• Form entries will need to be put into a PDF that I will be providing.
• This PDF will need to be downloaded once the form is submitted.
• Form submissions stored in back-end database.
• Would need a random unique reference code/number added to the PDF for each successful form submit.
