Ninja Forms Customization

I need some help with ninja forms. I've got the full package, and wanted to do two things. Create a custom pdf to display the scores. I've created the steps, added values to each answer, etc. I've created the results page that calculates score etc. So just need help to create a pdf that incorporates those values in a presentable PDF.

I'd ideally need examples of previous ninja forms work.
