Custom Wordpress Form/Calculation Tool

I require a calculation form building for Wordpress. The form is relatively basic and only a few variables. I estimate the project taking no more than an hour.

We are an agency listing this due to high volumes of work at the moment and are essentially looking for skilled developers to work with to deal with the over flow of work when required.
