I have a workbook template that has most of the functionality that I need but I need it modified to fit my requirements. I have 2 pdf forms right now that have dynamic drop downs. Meaning, the options in the 2nd drop down are dependant on the selection in the first. I will need 2 tabs on the user form, 1 for each of the pdf userforms I have. Basically, I want to switch from using this pdf form to an excel userform so that I can generate multiple at once. I do not need any help with the generation portion - only getting the userform to add each line to a worksheet table. I have attached the userform that I want to replicate -- although, it will take some modification to add the fields that I have.
