I need a bulletproof cocos2d UITableView, Modal alerts. Small project

This is a reasonably small project.
I need a sample cocos2d project that has the following:
1. Modal alert popup dialog using CCLayer/CCLayerColor
a. It **MUST** stop all interactions on everything underneath the modal alert; this must include CCScrollLayer*
* By which I mean, the CCScrollLayer stops moving altogether, you cannot touch it, move it or otherwise accept gestures onto it.
b. When the modal is closed/dismissed all interactions are back to normal, including CCScrollLayers.
c. The modal must launch only once when clicked from a CCMenuItem; you should not be able to click on a CCMenuItem multiple times to launch multiple modals
d. I do not want a UIAlertView; I want a custom CCLayer that acts like a modal pop-up alert. UIAlertView solutions will be rejected.
2. UITableView integration with cocos2d.
a. It must be integrated inside a CCLayer (ie, it could appear inside the modal dialog)
b. The UITableView must allow me to add CCSprites, CCLabelBMFont, CCLayers to it
c. The UITableView must give callbacks or something similar so that I know how to handle clicks on UITableView accessories or UITableView cells.
I have a sample project in Dropbox complete with some simple random sprites.
The sprites do not represent the actual game, they are to be used for testing purposes only.
An example of a UITableView inside a game is shown below. This is NOT my game, it is purely an illustrative guide.
