PHP Contact Form with document upload

Description
Experience Level: Intermediate
The job is to create a form using PHP, using the existing websites sidebars/logos etc, which sends a document through to an email address.
Access will be given to a sub-folder of the domain, where the person(s) working on it will have free access to develop/test. There are currently part-working forms etc on there already - you MAY be able to tweak the existing forms/PHP code and do it in five minutes.
You will be required to;
1) Review the form submittal page (or create new if you prefer).
2) Link the form to a PHP page which sends the document (along with comments, name, email address etc).
3) Test the form, making sure it can handle file types of .doc, .docx, .pdf, .rtf, .txt, and basically any MS Word/Word Processor/Document format.
4) Test that the emails are being received - you will be given access to the email account for the duration of the work.
5) Finally, I would require the person working on this to produce a crib-sheet (not masses of documentatin, half a page of bullet points is all I'm looking for) of how (and where) I can do the following myself;
a) Change the TO and FROM email addresses.
b) Change the host smtp server settings.
c) Change the smtp auth settings (i.e. username/password etc).
d) Change the domain the form works with (i.e. if this form is posted on www.rugby.com and I want to use it on www.football.com, what do I need to change).
e) List any other things that might be relevant or useful.
Going forward, although we are extremely small at present, we will without doubt be looking for a reliable remote web-resource to handle the things we can't do in-house. Long-term (12-24 months) this may lead to much bigger projects, this is a tiny job in relation to where we are on course to go. No worries if you can't or don't want to do that, right now we just need a contact form!
I am not fussed whether the person working on this is in Sheffield or Hong Kong, it's the technical know-how I am after.
