Dreamweaver Template Design required

I have designed my website pages in Indesign, and have created all the graphics. But I have only done a course in Dreamweaver and really need some help to create a Dreamweaver Template, so I can then create the pages and get online. As explained this is not a full web design project this is just a template required to help me do the rest. Anyone who can help me get online and kickstart my design business I would be extremely grateful. I attach a pdf of the sort of thing I am after.
