I need someone to format and design a sales page, which has already been written

Design details: I need someone to assist me in formatting and setting up a sales page. I have the writing and a sample of almost precisely how I need it to look.
I will also need someone to code, most likely into HTML as it will be placed on my autoresponder when complete ( I am not computer savvy as you can tell ).
I will need the job complete in a week. It is not complicated.
Also need the person to have ability to setup opt ins as I will have 3 to 4 throughout the sale page. This is crucial as they will also be linked by my autoresponder.

I need the individual to be able to give recommendations as well as follow instructions. I always listen to feedback.
Ideas for the visual style: will show template...
Where the design will be used: website and opt in page... sales page.
