I need a nicely-designed PDF document (text and pics) approx 5 pages

Hi - I'm trying to create a short PDF document (approx 4 pages) about me that I can send to potential clients when they request a quote. The document will have:

My logo and picture
Text about me
List of services
Testimonials from other clients
A few short samples of my work (text, plus logo/pic)
Contact details

I'd like it to be formatted nicely so that it looks professional and engaging. And I'd like the file size to be a reasonable size so that I can email it out.

I would send you the text, logos, images.
