I need a learning log book.

We are producing a learning log book for UK-CHINA students. We have the facts and contents done, need graphic design and illustration for the book. There are four topics in the book, for every topic, there need to be blank places for two letters from students themselves, rest if it with our facts and illustrations carefully designed. Thank you.
