Replacement & Novelty document creation - TWO options, so TWO quotes please

Design details: I require replacement documents that will need to be an exact copy, including holes punched.
I will provide a PDF scanned copy of the document. The printing should be in high quality, and cut to size with the holes punched per the original.

These are to be used FOR THEATRICAL, EDUCATIONAL OR NOVELTY PURPOSES ONLY.

I am hoping for a fast turnaround, with a proof send in 1 day. Unlimted amendments are required before the final version is agreed, attached, then the physical copies sent via recorded delivery.

There will be approximately 50 A5 pages in total.
Ideas for the visual style: Fake bank statements
Extra notes: Although fake, these will be in close up shots so need to be exact replicas per the PDF that will be supplied, except for the transactions that will be supplied per a spreadsheet (for data protection purposes).

I have now added TWO OPTIONS:
Option 1 is the more difficult A5 version. These include:
Holes punched in the appropriate place;
Printing on the reverse;
Cutting to size
Approx. 10 transactions per page.
Cover sheet is different to the rest.

Option 2 is the easier A4 version:
No holes punched;
No printing on the reverse;
Standard A4 PDF
Approx. 20 transactions per page.
Cover sheet is different to the rest.
