Book Cover

I have written a book about hospice services, to be published through createspace.

I would like someone to design a book cover for it, using imagery provided by me.

The design will have to be approved by a number of people, so we could be looking at a number of revisions, although we will hopefully minimise the work needed.

The cover must work / fit exactly with the createspace system, I do not want to be making revisions when I upload it.

I'd be particularly interested to hear from people that have experience of createspace and self publishing , with regard to formatting etc.
