Communication / Consultation
Please provide evidence of Communication / Consultation with staff in the last 12 months
eg copy of a safety memo, a set of minutes of internal meetings, signed and dated toolbox talk record etc.
To be acceptable please ensure the document is in your company name, contains the content of the talk and has a signature sheet to demonstrate staff have received the talk (to be signed and dated by team members)
Qualifications

Bathroom Installation - CSCS Card Kitchen / Bathroom Fitter / Plumbing qualifications

Plumbing - NVQ / C&G Plumbing

Concrete Works - NVQ Diploma in Concrete works

Carpentry and Joinery - NVQ Diploma in Wood Operations (Site Carpentry / Bench Joinery) / Apprenticeship

General Engineer/Fitter - NVQ Electrical / Mechanical Discipline

General Building Maintenance - NVQ Carpentry / NVQ Brickwork / NVQ Plumbing

Temporary Traffic Management - LANTRA Training / NVQ in Temporary Traffic Management (Construction) / NEW ROAD STREET WORK ACT

Risk Assessment / Method Statement (RAMS)

Please ensure the method statement is site specific and contains a detailed sequence of works

Please ensure the risk assessment identifies a number of risks (eg 3rd party safety, electrical, working at height, hot work, use of work equipment, manual handling, slips trips and fall etc) and for each risk that is identified there should be corresponding detailed control measures given.

Fall arrest Equipment
Please provide evidence of provision of Fall Arrest Equipment to employees by providing either:
• copies of signed and date Fall Arrest issue / inspection records dated in the last 12 months or
• copies purchase / hire invoices for Fall arrest equipment dated in the last 12 months.


Respiratory Protective Equipment
Please provide evidence of provision of Respiratory Protective Equipment (RPE) to employees by providing either :
• copies of signed and dated RPE issue / inspection records dated in the last 12 months or
• copies purchase invoices for RPE dated in the last 12 months.

COSHH
Please provide an example of a COSHH Assessment completed by your company in the last 12 months.
