Take a quick 10 minute survey with results used in an online game

We are creating an online game and are looking for male and female people to fill out a quick 10-minute survey, the results of which will be used to generate results for the game.

All you have to do is fill out a form.

It will ask you for a photo, this should be:

Just you alone
A head and shoulder shot
Taken in natural light
There should be space around your head
It should be a photo you have not used elsewhere online
Relaxed and informal

At the end of the survey you will have the chance to put in your freelancer username. This will ensure you get paid.
By filling in the survey you will agree to the use of your photo as a small thumbnail in our online game.
