We are aiming to create a database of Thomson Reuters 'COA' codes from a pdf document that we have. COA stands for 'chart of accounts' and these are the standard accounting codes that Thomson Reuters use to present their financial statements data.
They give users a big glossary which is almost 1,000 pages long, detailing how they collect data and providing a definition and description for each of the COA codes. I have attached this document entitled 'Reuters FundamentalGlossaryAugust2015.pdf' to this posting. I have deleted all non relevant pages from the pdf so you will find that the COA code definitions begin on page 1 and continue until the last page.
The job I need done is to copy every single one of these COA code descriptions into a google form so that I can easily make a database from it. There are approximately 1,200 COA codes to complete this task for. For each COA code, there are a number of standard fields entitled 'COA Code', 'Name', 'Units' etc. Each of these fields is a field on a google form which can be found here:

Please complete and submit one form for each COA code. I have also uploaded an image file called 'Example.png' for your reference, this should make it clear what goes in what box.
The basic fields are simple as they are short and correspond directly to a box on the google form. Do please take care to use the 'Eikon for Office Label' and NOT the 'Eikon for Office Field' in the relevant form field. The detailed description is a little more difficult in that the descriptions are sometimes very long, are split across more than one page and include bullet points etc. It is important that we capture all of the information in this description so please take care to copy all of it and to include bullet point formatting.
