Excel turned into a form that generates a report

I have a very basic excel spreadsheet with a number of rows - attached. The aim is to create a form that shows progress, using Red Amber Green ratings, the date that the task was completed, when it is to be reviewed and any comments.

I would like the form to be tidied up, made more pleasant to view and use.

I also want to provide a chart of progress showing actions in pie chart, red, amber and green etc.
