Data extraction and input

For the purposes of direct mail, I need someone who is prepared to visit named websites, identify chiropractors (as idividuals) by county (x50) then add the data fields (including name, clinic, location, and phone number) to an excel spreadsheet.

There are an estimated 300 -500 records.

The site is http://united-chiropractic.org and the data is easily located via the section 'begin care today'

Please provide cost per hour with time estimation and/or fixed project cost.
