We have an existing 5 page word document that our nurses complete when conducting a medical questionnaire on site for employees of our clients. This sheet is then mailed to our central office. We now require this word document converting to excel ‘master template’ so that it can be completed using a laptop (on site) then simply emailed to the office. This saves printing and completing it, saving time and money on posting it. We would require data validation and drop down menus for many of the answers. The fields that require free text will need to be text wrapped so that it is visible should a printed copy be required. The ‘master template’ will need to be protected so information cannot be saved over it and the completed questionnaires will need to save before sending. It would be ideal if each newly completed form could automatically be saved and sent as a pdf, so that records cannot be changed once completed.
