General information for the business: finance and insurance referrals
Industry: Business
Description of excel work: Create an excel form that captures information outlined on the attached example. The form needs to be very user-friendly, have smooth flow entering data between the forms and hopefully be compatible between Excel and Word (I'm not sure this is possible)?
The idea of this form is to be used as a referral form. ie. One person is referring an account to higher authority for acceptance. The form must have basic text fields and also some fields that calculate percentage changes, etc. It would also be nice if there could be a pie or bar chart that visually shows the changes.
An example is: the following revenue splits as percentages (would be helpful to see this visually)
Canadian - 45%
USA -30%
China - 5%
ROW - 20%
The form cannot be more than 2 pages, but must capture at least all of the info on the attached example. The fields don't have to follow in the exact placement as the example, but must be included and allow for easy navigation.
Extra notes: I need an application like form created that allows a user to enter various information in separate info boxes.
The idea is; Company A has to make a proposal to Company B in the form of a referral. The document would contain basic information fields that you would see on an insurance application such as:
Name of business, broker, Policy Period, Policy number, etc.
The form would also need to include some basic formulas to calculate percentage splits in revenues and then display them visually in a graph. Please be creative. The whole idea is that the form be super easy to flow through, include all the fields required and hopefully be compatible between both Word and Excel (meaning, one could save the file as an Excel doc or Word doc) - if possible.
I've attached a sample, but the reason I'm not using this one is because it is too cluttered and user-friendly. Please don't copy this example, but use the info on it and be creative in the form. It must be no more than 2 pages.
