Title:PHP Site Registration forms need programming correctly

Location:Anywhere

Job Description

I previously advertised this job back in May, but held off due to the current programmer stating that they would get the issues with the forms fixed.

There are three forms which are all similar, all are supposed to send details through to three different email addresses. However no details go through!

The three forms can be seen here:

http://www.ennerdalehouse.co.uk/registration_news.php5
http://www.ennerdalehouse.co.uk/registration_referral.php5
http://www.ennerdalehouse.co.uk/registration_cosmetic.php5

Additionally I would like to have a thank you screen that comes up after your details have been submitted.

Also we would like to include a facebook link on the homepage, a pdf layout will be supplied along with the graphic file.

I assume that mail forwarders have been setup on the server to forward any emails sent via the forms to the client's email addresses.
