PHP & MYSQL database designed and coded with various functions

Our task as required:

We are looking for a web based database to be coded in PHP and stored in a MySQL database ONLY. PHP / MYSQL ONLY.

The database will require two front ends:

1). User login and entering the information.

Design a Login page to be used with a smart phone / iPad [ 300x200 preferably ]

Login page to have the following:

Operator - Drop down list with names [ that can be added / removed in the admin backend panel ]
Password - Password
Operator NUMBER: The operator's ID number, ie 029394 [ HAS TO MATCH THE OPERATORS NAME, SPECIFIED BY ADMIN IN BACKEND ADMIN PANEL ]

User enters username, password and number. All 3 must correspond with one another. For example:

User: 1234
Pass: password
Number: 019191

LOGIN OK!

Any failed logins to be emailed immediately to admin email.

User will be brought to a page with two links - PREVIOUS or ADD.

PREVIOUS will show the user's previous entries.

Add will bring user to the add information to database. The user is then brought to page where all information is mandatory / required. Once the user hits submit, a confirmation box "ok to submit" with a yes / no button.


User will then be brought back to his submitted information, in a simple table view with a check box.

User can view previous information edited but can not delete anything or edit anything. [ Simple echo command for example ]

User also has the option to email each entry if the CHECK BOX is ticked [ emal address to be specified in admin panel ]

User also has a logout button to end his / her session.

User not to be logged out unless he / she clicks end session.

2. Admin login.

The second page will be for the administrator to:

Update the information
Run reports
Add users
Change users
Delete users
Change passwords
Change numbers
Set email address
Export reports to excel, filter data etc.

Admin will also have the facility to change ALL fields, ie:

Passwords,
Users,
Numbers

Basically, complete control over the front end for the user / backend for the database.

The database fields will be as follows:

Column A - Number
Column B - Name
Column C - Phase
Column D - Spaces
Column E - Calendar Date
Column F - Time
Column G - people with stamps
Column H - staff stamps
Column I - people Warned
Column J - Total people
Column K - % of users to Total Spaces
Column L - Variance - Spaces / people
Column M - users done
Column N - Operator [ user ]
Column O - Comments

PLEASE NOTE - THE COLUMN NAMES [ Number, Name, Phase.. ] will be changed, they are just an example.

Column K - This field should automatically calculate J & D, ie 10 people, 20 spaces, column k = 50% - it should not be editable by the operator.

Column L - This field should automatically calculate J & D, ie 10 people, 20 spaces, column L = 10 spaces left - it should not be editable by the operator.

All fields are mandatory for the user to enter information in.

Columns A, B, C and D will all be drop down lists which can be tweaked in the backend admin panel.

Reports will be required for all, for example.

On E [ date ] at F [ time ], there was K [ 25% ] of the D [ spaces ] left in [B] [ Name ].
