Hi, I'm would like to create a form having three pages/stages.

Page 1 - Basic form with text boxes, radio buttons, drop down selections and a couple hidden text boxes where they appear if you select yes in a specific question.

Page 2 - Optional image file uploading form. Max 5 images. I would like to have them uploaded to the server in a specific folder. Also sent them attached in an email at the form submission.

Page 3 - Basic contact info, ie name, email, phone. Submit button and email is sent with all the answers and images (if any) attached.

All data need to be validated preferably separately for each form page and if any error then display it on the page. I would like to have php validation. Not javascript etc.

Not interested in styling the form with css atc. Html is the last thing to worry about.

Hi,

This is jeason, I had gone across the requirement and seems to be fine for us. I have some queries regarding the application

1. What are file types (jpeg,gif,bmp,tif) we need to upload.
2. Is there any document need to upload.
3. Should we integrate any further ajax script for file uploading.
4. Is there any max or minimum file uploading restriction.
5. How many users are interacting with the application.

Kindly revert back the updates for the above mentioned.

hi jeason, thnx for your message.

1. The types are any images, jpeg, gif, bmp, png, any others?
2. No document or any other file type to be uploaded.
3. No other integration, just a basic and simple click a button, browse for a picture, hit upload.
4. There is a maximum of 5 images for uploading.
5. It's not going to be a member form, anyone can access the form and use it.
