Project Scope:
Repair broken code for our online enrollment form with seemless integration through CyberSource for Credit Card processing along with keeping style from our website consistent throughout the form (new website - some work has been completed in switching over to new style sheets).

Currently the form only processes successfully for pre-registration. All other enrollments types calculate a $0 tuition charge and creates an error message within Cybersource.
Update current form to include:
* Current Pricing Options
* Specials Pricing Page
* Language Survey
* add last line of Person completing Enrollment Form.
* Agreement acceptance for tuition, refund, withdrawal and privacy policies (http://www.cmaconnect.org/limesurvey/index.php)
* User Interface for pricing and course selection updates
