Design a PHP Form with printouts -- CodeIgniter

You have to create a single form with some fields.

This can be repeateable fields, so there's a need of a look up query to fill in the field automtically.

Then the form have to be printed 3 times.

Simple Example (not all the fields are in here

Owner Of the Car : 'Name1'

Type of the car : 'Hyundai'

Serial Number of the Car : '112233'

New Owner of the car : 'Name2'

Print

In a second form Kurt again can be the oner of the car, so we could look his name & address.
