Database Creation

I would like to commission the creation of a database of health and beauty businesses in the UK (ie hairdressers, beauty parlours, nail bars, dentists, cosmetic surgery clinics, alternative therapy clinics etc etc).

I am looking for around 5,000 entries with an even geographical spread and the information included should be:

- name of the business
- address, including postcode
- contact number(s)
- email address
- website

The only stipulation is that this database cannot be scraped from another online source.
