Job Description
I am looking for someone to do the following to my existing website:
1. I have an online forum on the website where people can post questions etc and I need some security put on the forum entry form as the forum is getting bombarded by junk entries. i.e. captcha.
2. Identical to above but on product review forms.
3. To create a stand alone online form for VAT exemption, identical to an existing form which is only accessible through the checkout process, which posts the results back to my email address.

If you are not sure of the system/package that your website uses, the website URL (link) should suffice...
I am confident that I can add CAPTCHA functionality to any website and have done this many times before, however, could you please explain in a little more detail your third task:
3. To create a stand alone online form for VAT exemption
For example, what information should be within this form? Where exactly should this form appear (within the checkout process?)? and does the form need to be stored in the sites database or just emailed? Does the information entered in this form have any effect on any aspect of the website itself such as change prices etc?

the VAT form exists already but is only accessed when a customer goes through the checkout process after placing an order. I need exactly the same form but as a stand alone page the I can direct customers to if they place an order over the phone. The results are then emailed to us.

J. M. on 04/12/2010 23:07captcha etc will not entirely eliminate junk appearing in your forums. You need to be vigilant with your moderation and also perhaps vet users who register from strange places or show little activity.
Here are some practical tips on staying safe
- Never pay any money to Clients and or provide sensitive details like your Bank information, Credit Card or Passwords
- Never accept a Job if you have been directed away from PeoplePerHour to another website
- Be cautious when providing your personal contact details (email, telephone number, instant message ID etc) before your Bid is accepted
- If you have been asked to send samples, put a watermark on them or send low resolution examples to avoid being exposed and not getting paid
- Don’t start work before your Bid is accepted and your Escrow deposit is made
- Keep your Escrow account topped up and ensure that you use the Private Message Board to track all pertinent details about the Job e.g. timelines, deliverables etc
- Before you supply final works to Clients, ensure you have adequate funds in Escrow
Remember: if something looks too good to be true, it probably is!
