Website Conversion to Joomla

I have a site the needs to be converted to Joomla and in the process 4 minor issues need to be fixed.

The 4 issues include the following:

1. There are 3 application forms that can be completed online. For each of these I need a PDF designed that can be printed to an applicant can complete it manually. The button for print is already there so this just need to set the PDF up to be printed.
2. On one of the forms we need to add a check box and based on that box, hide or show a group of input fields. The inpit fields are already there so we just need the hide/show functionality added.
3. There is a box on the pages that display a set of rates. It needs to be dynamic so the rate can be changed by the user as often as necessary. Jooma's content management functionality will do fine but the box needs to be redefined as editable content rather than a jpeg.
4. The home page has a layout that should be consistently applied to all other pages. Right now there are some elements that are not being carried through to the other pages.

This is an urgent job that should be quick to do but must be complete by the end of this coming week. I will provide the successful bidder with two zip files, one containing all of the site material and the other with the database.

This is a fixed price job and I will be making an immediate assignment.
