Simple Flash with database

The project requires to create a simple interactive flash file.
On the first page users will select options (3 sliders, one imput) and click submit. The entered information will be compared with table of results in the database and then will be shown one of the 3 results supported with a short animation. The entered data needs to be captured and available for client to reach/extract. The flash will also include a option to choose between square metres and feet.
The visual design is done in illustrator, the table for results comparison is an excel file.
The project needs to be done by 26th August including testing on client's server. We are looking for a competitive quotes.
